Purpose and Outcome
            The purpose of the assessment plan will be that the students in 1st grade will understand their hygiene. They will understand how to take care of themselves and what they need to know to take care of themselves on a daily basis before they leave their house to go to school in the morning. They will understand that they have to follow a daily routine to keep their hygiene together and be clean.

The learning Outcome
            The learning Outcome that the students will understand is how to take care of themselves and know that it is a routine that they must follow every day. They will learn the importance of being healthy and have a healthy habits that they need to do and can teach their siblings. The students will understand why it is important. They will also know when they see an item for healthy hygiene they will be able to recognize and know how to apply it to themselves.

Assessment context
            The assessment will be one or two sheets of paper where the students will have five questions where they will have to match up the word to the picture by drawing a line to each other. In the front of the classroom will be actual samples of a toothbrush, dental floss, soap, wash cloth and comb. The words that will have to be matched up to the picture are teeth to dental floss, hair to comb, toothpaste to toothbrush, hands to soup, and face to wash cloth.

            The next part of the test will be an essay where students will have to draw a picture and answer the question. For example; draw a picture of a comb. It is important to __________. The answer can be “It is important to comb your hair”, or “It is important not to share your comb”.

Testing Constraints
            The students will have learned all of this information in the classroom. They will have had practice sheets given to them in class for discussion and homework sheets to do at home as well. They will have 45 minutes to complete the test and will be able to raise their hand to ask the teacher for help if they are stuck on any part of the test. They will also be able to use the bathroom or get a drink of water during the test because there will be more than enough time for the student to finish the assessment. After they are done, they will be able to sit at the art table and color silently until all the students have finished the assessment.
